Factors Leading To Congenital Hearing Loss in Patients Coming To a Tertiary Care Hospital in Karachi: A Retrospective Study of 87 Patients

Background: The sense of hearing is an integral part for the development of speech and articulation of a child. In Asian countries the noted prevalence of Congenital Hearing Loss (CHL) is 1.6 in 1000 as compared to 1 in 1000 in western countries, making it the most common sensory disability in the world. The trend of consanguineous marriage is an important factor particularly in Pakistan, leading to higher number of reported cases. Objectives: i. To find out major genetic and environmental factors leading to CHL in Pakistani population. ii. To identify families with the possibility of genetic cause running in generations leading to CHL. iii. To educate patient families about CHL. Material/methods: This study is based on data retrospectively collected from 87 patients identified with CHL who came to ENT clinic in Aga Khan University Hospital during the period of January 2016 to March 2016. After patient consent, a research questionnaire was filled. In the cases with positive history of hearing loss in other family members, pedigree chart was developed. Results: Patient presentation ages ranged from 01 month to 46 years with the median of 04 years (mean = 6.0 years; standard deviation = 6.6), 49 patients (56%) were male and 38 (44%) were female. 87% cases having a profound degree of hearing loss. 63% cases were a product of consanguineous marriage. Family history was found positive in 48% cases suggesting a hereditary cause. 28% cases had a preceding history of severe illness, neonatal jaundice and meningitis being the most common whereas no known causative factor was found in rest of the cases. Along with hearing loss, delayed milestones were reported in 33% cases. Conclusion: Consanguineous marriages were more common in CHL patients, genetic testing to identify specific gene involvement in Pakistani population is essential to recognize the exact cause.
