Title: Update: The Baby Sound Program - A Model Program for Infant Hearing Screening in the Medical Home
Primary Track: 4-Medical Home
Keyword(s): well-baby screening community clinic medical home

Abstract:

Early and periodic hearing screening (EPHS) in the Medical Home is the new frontier in early detection and follow-up for young children with hearing loss. As an adjunct to hospital-based newborn screening programs, EPHS in the Medical Home has the potential to greatly reduce loss to follow-up and to systematically identify children with later onset and progressive losses. In addition, the Medical Home can provide continuity and coordination of care for children with identified hearing loss and regular monitoring of hearing health as a component of well-child pediatric care. Baby Sound Check(TM) is a model EPHS program developed in collaboration with community health clinics that serve low-income and at-risk children in the greater Los Angeles area. Baby Sound Check is designed to build EPHS capacity in low-income community Medical Homes by providing equipment, training, screening and follow-up protocols so that pediatric teams can incorporate hearing screening into routine well-baby visits. In this way, children who have been lost to newborn screening and/or follow-up can be identified, as well as those with post-natal onset and progressive losses. The screening protocol consists of otoacoustic emissions, tympanometry and acoustic reflexes administered by Medical Assistants and interpreted by physicians. This presentation will report on the first year of implementation, including referral frequency and outcomes, protocol revisions, implementation challenges and successes, and impressions from involved medical personnel at the targeted health clinics.
