Title: Medical Home Viewed through the Eyes of Families
Primary Track: 4-Medical Home
Keyword(s): Medical Home

Abstract:

MCHB programs support a medical home for all children. North Carolina decided a tool was needed to promote and facilitate implementation of the medical home principles, especially for the families of children with hearing loss. The term “medical home” is often unfamiliar to families and difficult to define clearly. The complete set of principles that define the medical home approach can be overwhelming to both families and providers. The intent is to use this video in a variety of trainings and discussions with families and professionals across the state. Six families of children with hearing loss are depicted in the video. These families share their personal stories about how they forged a successful partnership with their health care providers to create a medical home. Each story highlights a different way to empower families and to promote shared decision making and coordination with families. The purpose of the video is to promote family-centered strategies as critical to the success and value of having a medical home team for a child with hearing loss. The video was created as part of North Carolina’s MCHB Early Hearing Detection and Intervention grant to address monitoring hearing loss within the medical home. National organizations have been promoting the use of individual stories to give more meaning to an issue or cause. Our state Title V agency sees the use of video to tell family stories as a promising practice to increase the understanding and use of key medical home elements.
