Abstract:

The Arizona Chapter of the American Academy of Pediatrics (AzAAP) partnered with AzEHDI, including the Arizona Department of Health Services, Office of Newborn Screening and The EAR Foundation of Arizona to raise awareness about Loss to Follow-up/Documentation, including Medical Home responsibilities in meeting the 1-3-6 EHDI goals. In Arizona 98% of all infants are screened by one month of age and more than 80% of children who fail the newborn hearing screen return for a second screen. The area of greatest breakdown is for infants completing the diagnostic evaluation by three months. With more than 50% loss to follow up or documentation at this stage, strategies targeting high loss to follow up by regions regions-was the focus. Pediatricians are the most influential to parents deciding to follow up on failed newborn screening results and need ongoing opportunities for further education about how they and their office can be 'EHDI ready', including awareness of the screening and diagnostic status of their patients and what the next steps need to be to meet the EHDI goals. The child's medical home provider plays an important role in ensuring that families meet the 1-3-6 goals. Each member of the pediatric office, including office staff, referral coordinator, medical and physician's assistance play a critical role but are not aware of the 1-3-6 EHDI goals or overall newborn screening best practices including those needed to reduce loss to follow-up. Objectives: The goals of this project were 1) Reduce loss to follow-up/documentation, 2) Educate medical practices to be 'EHDI ready” 3) Help practioners understand how to know the screening status of infants, the risk factors for late onset and progressive loss, the next steps needed to ensure early identification of children who are Deaf or Hard of Hearing, 4) Survey to measure outcomes.

      Randi Winston has been a consulting audiologist for The EAR Foundation of Arizona (EFAZ), the Arizona Department of Health Services and Arizona's State EHDI Program since 1998. Her role has primarily involved working with hospital based newborn hearing screening, early childhood hearing screening programs statewide. She continues to provide ongoing training and technical assistance with an emphasis on program quality. She serves on the National Center for Hearing Assessment and Management (NCHAM) Technical Assistance Network, assisting states in Region X in their EHDI efforts.

      Lylis Olsen is the State EHDI Coordinator in Arizona. She is a pediatric audiologist with 25 years of experience. She also has a Master's in Public Health and has 21 years experience in state newborn hearing screening programs. She works for the EAR Foundation of Arizona and also manages the HEAR for Kids program.

      With 15 years’ experience in technology, program development, and training, Sondi Aponte is the Quality Improvement, Education and Outreach Manager with the Office of Newborn Screening (ONBS) program at the Arizona Department of Health Services. In that capacity, she develops quality assurance measures, monitors program outcomes, oversees hearing grants, and is the project lead on the CDC Data Integration grant. Program quality improvement responsibilities include data evaluation and analysis, report creation and overall data integrity. Partnership development and enhancement are crucial components of her job, representing the ONBS with The Az. Academy of Pediatrics, The Arizona Perinatal Trust and The EAR Foundation of Arizona (EFAz) among others.Sondi develops and implements training materials and educates clinical providers and families on newborn screening best practice.
