EHDI Program and Medical Home Partnership: Promoting Appropriate Follow-Up Through “Just-In-Time” Learning
A large percentage of Louisiana infants receive hearing re-screens, following discharge, through the Medical Home. Current goals in both the MCHB and CDC grants focus on increasing awareness and education at the Medical Home level. Through efforts such as: 1) direct mailings, 2) physician visits, 3) “Lunch & Learns” and 4) “Just-In-Time” Learning, we have made substantial progress in meeting these goals and objectives. The EHDI Program and Medical Home Partnership is benefiting all stakeholders by enhancing a positive EHDI role in the community, educating physician offices in appropriate screening techniques and positively impacting our lost to follow rates. We will discuss information gained from a recent survey conducted with Louisiana PCPs. This presentation is a follow-up to a previous presentation at the 2010 EHDI Conference: “Hearing Rescreening in the Medical Home – A state Model to Improve Follow Up Reporting”.

Wendy Jumonville

Wendy A. Jumonville received her Master of Science degree from Purdue University. She has worked as a Pediatric Audiologist at Denver Children’s Hospital, The Louisiana School for the Deaf, and is currently on faculty at LSU BR in The Department of Communication Sciences and Disorders. She currently provides contract services to EHDI as an Outreach Specialist.

Kay Darr

Valarie Kay Darr is an audiologist who is currently working as an Outreach Specialist for the Louisiana Department of Health and Hospital Early Hearing Detection and Intervention (EHDI) Program. Kay has worked in several ENT offices, private practice audiology clinics, and as both a hospital staff audiologist and hospital department manager. She earned her B.S. in Communications Disorders from the University of Texas at Austin in 1984 and her M.S. in Audiology from North Texas State University in 1987.
