A Small Step to Improve Loss to Follow Up
The NICHQ (National Institute for Children’s Healthcare Quality) Project investigated whether small steps of change would decrease loss to follow up rates. One suggestion resulting from this project consisted of scheduling a rescreen appointment before baby and mother left the hospital. The University of Michigan has incorporated this idea into newborn hearing screening protocols. As a tertiary care center with a Level III NICU, babies from a variety of locations are screened through the EHDI program. In reviewing annual statistics of infants referring on initial screen, over 10% of families declined further testing at the University of Michigan and indicated a preference for testing to be completed closer to their home. Since there are a limited number of diagnostic centers throughout the state, this raised concern as to whether families could identify and obtain timely follow up for these infants. This presentation will review a “small test of change” focused on decreasing loss to follow up of this population.
- Discuss how “Preferred Diagnostic Center” list was developed in Michigan
- Describe how Diagnostic Center List is used to assist families in identifying the closest center to their home following referred screening at the University of Michigan Medical Center.
- Judge if similar strategy would be effective within own practice.

Angelique Boerst is an audiologist at C.S. Mott Childrens Hospital within the University of Michigan Medical Center. In addition to completing hearing assessments of children, she has been a team member of the Sound Support Program which provides outreach and support services to families of children who are deaf/hard of hearing.

Dr. Carlson works at CS Mott Children's Hospital at the University of Michigan Medical Center. In addition to assessment and management of pediatric patients, she oversees the EHDI program at the University of Michigan. She is also a member of Michigan EHDI Advisory Committee.
