Illinois’ Efforts in Reducing Lost to follow-up in Chicago’s Auburn Gresham Community Area through Community Partnerships and Quality Improvement Activities
This presentation will provide an overview of the Illinois’ EHDI Learning Community in the Auburn-Gresham Community Area in the city of Chicago. The presenter will discuss the selection of the target area and the organization, membership, and activities of the Learning Community. This presentation will describe the quality improvement projects that have been conducted by the Learning Community, as well as the administrative support provided to the Learning Community. As a result of these efforts, Illinois has noticed a reduction in the numbers for infants with suspected hearing loss who were lost to follow-up. Although Illinois’ EHDI program has been successful in creating and sustaining a Learning Community, several challenges in engagement of pediatric healthcare professionals and families remain. We anticipate that this presentation will be helpful to states that continue to struggle with keeping their Learning Community members engaged and face challenges in decreasing lost to follow-up.
- Participants will be able to describe Illinois’ EHDI learning community including: the recruitment process; benefits of participation; challenges, barriers and lessons learned.
- Participants will be able to describe how a state may use quality improvement activities to strengthen service delivery to children who are in need of follow-up and children who are deaf or hard-of-hearing
- Participants will be able to identify strategies that Illinois’ EHDI program has used to reduce lost to follow-up in the Auburn-Gresham community in Chicago.

Esha Phatak graduated with a Master of Public Health degree in 2011. Ms. Phatak currently works with the Illinois's Title V- Children with Special Health Care Needs program that is housed within the University of Illinois at Chicago- Division of Specialized Care for Children (DSCC). Ms. Phatak has been the HRSA EHDI Coordinator since May 2015 and works closely with the Illinois Department of Public Health in providing oversight for the statewide EHDI program components including reviewing and assessing the screening, diagnosis, and intervention procedures. Additional responsibilities include: formulating standard operating procedures for EHDI program activities; assessing program data and assisting in writing grants and required reporting documents and; participating in statewide interagency conferences and outreach activities to improve systems of care for children with special health care needs and their families. Prior to her work at the DSCC, Ms. Phatak worked for the Iowa DPH EHDI from 2010-2015.
