Gaps in Pediatric Hearing Care: Sickle Cell Disease

Sickle cell disease (SCD) is a hematologic disorder with genetic origins that can manifest in many different ways throughout the body, including hearing loss. Symptoms of sickle cell disease in children can mimic attention deficit disorders or learning disabilities, similar to how symptoms of hearing loss can masquerade as presumed inattention or defiance. Sickle cell disease is broadly screened for at birth, similar to universal newborn hearing screenings, though a lack of guidance exists on appropriate audiologic monitoring schedules. Symptoms of sickle cell disease, including hearing loss, can manifest by school age (4-6 years) or earlier, making timely identification and treatment of hearing loss in this population vital for ensuring a child’s full academic potential can be realized. A primary objective of this literature review is to further explore proper audiologic monitoring protocols for patients with sickle cell disease. I aim to provide a greater understanding of the prevalence and presentation of hearing loss associated with sickle cell disease. Additionally, promoting increased familiarity among physicians and other healthcare professionals regarding the connections between hearing loss and sickle cell disease could lead to higher rates of referrals for audiologic evaluations in this population. Ideal learner objectives include the ability to explain the basic effects of sickle cell disease on hearing, recognize when hearing screenings are appropriate for sickle cell patients, and determine an audiologic monitoring schedule based on the needs of the patient.

Peter Shireman is completing the fourth year of his doctoral program in audiology through the University of Illinois and is the current audiology extern at the Kennedy Krieger Institute in Baltimore, MD. Peter has a B.S. in psychology from the University of Wisconsin-Madison and is planning a career in pediatric audiology following graduation in May of 2023.

Amy is the Director of Audiology at the Kennedy Krieger Institute. She has worked as a pediatric audiologist for over 20 years. She received a B.S. in Hearing and Speech Science from Towson University and an M.A. in Audiology at the University of Maryland. She also received an Au.D. from Arizona School for Health Sciences.
